1. Consent to Telehealth
By submitting a secure intake, scheduling a virtual visit, or participating in telehealth communications, you consent to the use of telehealth technologies for review, consultation, care coordination, and follow-up.
Telehealth may include secure forms, video, phone, messaging, email, electronic records, and remote provider review.
2. No Emergency Care
BioFuelHQ is not an emergency service. If you are experiencing chest pain, trouble breathing, severe allergic reaction, suicidal thoughts, stroke symptoms, severe bleeding, or another emergency, call 911 or seek emergency medical care immediately.
3. Provider Review Required
Submitting a form does not guarantee approval, treatment, prescription, medication access, product access, or eligibility.
A licensed provider or authorized clinical reviewer may determine that additional information, labs, an in-person visit, or referral to another provider is required.
4. Risks and Limitations
Telehealth has limitations. A provider may not be able to perform a physical examination, may need additional records, and may determine that telehealth is not appropriate for your situation.
Technology failures, connection issues, privacy risks, or incomplete information may affect telehealth communications.
5. Patient Responsibilities
You agree to provide accurate, complete, and current information.
You agree to use a private location when possible and protect your own device, password, and email account.
You agree to notify BioFuelHQ or the provider team if your health changes or if you experience side effects or adverse events.
6. State Availability
Services may be limited by your state of residence, provider licensure, pharmacy availability, and applicable law.
BioFuelHQ currently focuses on patients located in Florida unless otherwise stated.
