Treatment: We may use or share health information with licensed providers, pharmacies, labs, or care team members involved in reviewing your request or coordinating care.

Payment: We may use or share limited information to process payment, verify payment status, or resolve billing issues. Payment is handled through a third-party payment processor.

Health Care Operations: We may use information for quality review, compliance, credentialing, vendor management, auditing, training, and business operations.

Legal Compliance: We may disclose information when required by law, court order, subpoena, audit, investigation, public health requirement, or regulatory request.

If a breach of unsecured protected health information occurs, we will provide notice as required by applicable law.

6. Complaints

You also may file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.

We will not retaliate against you for filing a complaint.

7. Effective Date

This Notice is effective as of July 22, 2026.
